Investigating the Factors Affecting the Performance and Sexual Satisfaction of Hysterectomized Women in Reproductive Age Referred to Shahid Mofatteh Clinic in Yasuj, Iran

Abstract

Background & aim: Sexual dysfunction after hysterectomy is a common problem that occurs in most women after the operation. Hysterectomy is the most common surgical procedure in women and sexual dysfunction in reproductive age is highly prevalent. Therefore, it seems necessary to investigate the complications of various types of hysterectomy in Iran. Therefore, the aim of the present study was to determine and investigate the sexual function and sexual satisfaction of hysterectomized women of reproductive age who referred to Shahid Mofatteh Clinic in Yasuj, Iran, from 2019 to 2022.


Methods: The present descriptive-analytical study was conducted in 2019-2022 on 66 women of reproductive age who underwent hysterectomization. The present study was conducted through non-random and accessible sampling method using interviews and filling out two standard questionnaires during two periods of 3 and 6 months after the operation. The data collection tools included a demographic characteristics questionnaire - demographic variables, a sexual function questionnaire (FSFI), and a questionnaire related to sexual satisfaction. The collected data were analyzed using SPSS software and descriptive (mean and standard deviation) and inferential (Chi-square, Mann-Whitney) statistical tests.

Results: In the present study, sixty-six women of reproductive age (15-45 years) who had undergone hysterectomy were examined. The mean age of the participants was 3.62 ± 41.77, the age of marriage was 5.79 ± 18.42, the number of children was 1.22 ± 2.88, and the age of the participants' spouses was 5.62 ± 47.09. 46 women (69.69%) had TAH hysterectomy, 12 women (18.17%) had TVH hysterectomy, and 8 women (6.1%) had supracervical hysterectomy. A statistically significant difference was found between the types of hysterectomy with sexual function (p=0.045) and sexual satisfaction (p=0.043), such that abdominal hysterectomy had lower sexual function and satisfaction. Also, 44 ​​women had their ovaries (unilateral or bilateral) preserved during the operation, and this had an effect on the behavioral and emotional aspects of sexual satisfaction (PISQ), sexual desire, and dyspareunia in sexual function (p=0.0135). The most common causes of hysterectomies were abnormal uterine bleeding (AUB) (40.9%), cesarean section (18.2%), and then fibroids (16.7%). The education of the wife and husband, type of employment, type of housing, did not have a significant difference with the sexual satisfaction of the patients, but there was a statistically significant difference in the amount of income, age and age of the wife and ovarian status.

Conclusion: The type of surgery (vaginal and supracervical) and with/without oophorectomy (unilateral or bilateral) and the amount of income had a direct effect on the sexual performance and satisfaction of women of reproductive age. By identifying some factors affecting sexual problems after hysterectomy, a solution can be found to adjust this disorder and increase the quality of married life of these people after the operation.
