Aortic arch abnormalities in patients with tetralogy of fallot

Aortic arch anomalies in patients with tetralogy of Fallot
 
Abstract:
Introduction:
Tetralogy of fallot (TOF)  is one of the most common cyanotic congenital heart diseases. Right aortic arch is seen in 25% of the patient with TOF. Moreover, right aortic arch is associated with several heterotaxy syndromes and abnormal origin of aortic branches. This is important for the surgeon to know if there is any aortic arch or branch abnormalities before surgery so this could help him/her to draw a map way with the least complications during and after operation.
Materials and methods:
In this retrospective study, 332 cases  belong to the patients with TOF who had been referred to Shiraz  Namazi/ Shahid Faghihi hospital  cath lab for pre-operative diagnostic angiography were reviewed. Reviewed items were including demographic data, echocardiographic reports, and angiographic films which had been recorded on the CD or HIS system. In all patients, direction of the aortic arch and any abnormal origins of aortic branches were recorded and finally all data were analyzed with software SPSS version 19.
Results:
332 patients including 20 adults and 312 children (<=18 years) from age 1 month to 36 years were studied. 208 males and 124 females (23.2%) had right aortic arch. In one them with right aortic arch, interrupted arch was reported too. Contribution of right aortic arch between to sexes did not show significant difference (22.1% and 24.9% of males and females, respectively)(p=0.5). In this study, abnormalities of the aortic branches were not seen.
Conclusion:
Right aortic arch is seen in the noticeable percent of the patients with TOF and abnormalities of the aortic arch may be associated with it. To know how the aortic arch and branches lay toward its pathway could help the surgeon to have a best strategy before surgery.
