Comparative study of the effect of teach back teaching method and short message on adherence to the treatment regimen in patients with hypertension

Background and Objective: High blood pressure is a major global health issue, and treatment adherence is a key factor in its management. Educational programs play a vital role in controlling hypertension. The Teach-back method—a face-to-face Q & A approach—helps patients explain what they have learned in their own words. Meanwhile, SMS-based education can also provide valuable healthcare support. Therefore, this study was conducted with the aim of determining and comparing the effect of teaching using two teaching methods, Teach- back and short message, on adherence to the treatment regimen of hypertensive patients.

Methods: This study is of quasi-experimental type, and the research population consists of all patients with primary type hypertension who referred to the health centers of Cheram city in 2023. The number of 176 patients eligible to enter the study was selected through convenience sampling and based on random block allocation, they were assigned to two intervention groups, teach- back and Short Message. The data were collected by using a demographic data form, a 54-question questionnaire on adherence to the treatment of hypertensive patients, and blood pressure measurement of patients before and after the intervention. The collected data were analyzed using SPSS version 26 software and descriptive statistics and Chi-score, independent t-test and paired t-test.

Finding: Before the intervention, there was no statistically significant difference between both groups of research surveys (P<0.05). After the intervention, there was a statistically significant increase in the adherence score to the treatment regimen in the Teach- back group compared to the text message group with the mean 4659.21±183.27 and 4434.41±171.01  respectively (P<0.05).  

Conclusion: Both teach back and short message teaching methods increased adherence to treatment regimen in hypertensive patients, but Teach- back method had a greater effect. Therefore, it is suggested that the members of the health team should consider the implementation of both training methods for those suffering from high blood pressure and use this useful and beneficial program.
