Planimetric and biomechanical study of local effect of Cineole on full thickness wound healing in rat

Background and Aim: Wound healing is the interaction of a complex cascade of cellular and biochemical actions leading to the restoration of structural and functional integrity with regain of strength of injured tissues. The aim of the present study was to evaluate the wound healing activity of Cineole on full thickness induced wounds in rat.
 
Material and Methods: The present study was a prospective experimental study. Assessment of the healing process was based on planimetric and biomechanical studies within a 21-day period after surgery. Forty rats were used in this study. For Planimetric study 20 rats were randomized into five groups of 4 animals each: Control group with creation of wounds and no treatment, base formulation group with creation of wounds and application of base formulation ointment, treatment group (E1) with 0.25 mL of Cineole in ointment, treatment group (E2) with 0.5 mL and treatment group (E3) with 1 mL of 1,8-Cineole in ointment. All the wounds in all experimental animals were rinsed daily by 10-ml sterile normal saline for 10 days. The percentage of wound area was measured in each group on days 3, 6, 9, 12, 15, 18, 21 after photography using a digital camera. For biomechanical study 20 rats were randomized into five groups of 4 animals each the same way. On day 9 the samples were taken for biomechanical assessments including yield point, ultimate strength, maximum energy stroed and stiffness parameters. Regarding wound contraction, the values were analyzed using ANOVA Repeated-Measures and in biomechanical studies the values were analyzed using one-way ANOVA. The P<0.05 was considered as level of difference.
 
Results: In E2 group biomechanical indices of yield point (1.49±0.10), maximum tensile strength (1.64±0/11), maximum stored energy (1.74±0.32) and stiffness (1.74±0.11) in healing tissue were significantly different from those of control, E1 and E3 groups (P=0.002). Following analyses of results of application of ointments containing Cineole, it was observed that application of 0.5 % ointment of Cineole on days 15(5.67±1.10), 18(2.7±0.40) and 21(0.30±0.38) reduced the wound area earlier than other treatment groups. So that, 0.5 % ointment of Cineole in E2 group (0.30±0.38) sh0wed more accelerated contraction of wounds compared to other groups (P=0.002).
 
Conclusion: 0.5 % ointment of Cineole could be useful in open wounds due to acceleration in wound contraction and decrease of time in filling the wound with the granulation tissue.
