A Review of the Genetic Diversity and Immune Escape of SARS-CoV-2

Background & aim:  The outbreak of 2019 novel coronavirus disease (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), started in late 2019 in Wuhan, China, and in a short time, it has spread rapidly all over the world. SARS-CoV-2 has evolved to display particular patterns of genetic diversity in the genome across geographical regions. These variations in the virus and genetic diversity in human populations can determine virus transmissibility and COVID-19 severity. Genetic diversity and immune differences in human populations could be the driving forces in viral evolution and escape from the immune system. The aim of this study was to review the research conducted on genetic changes and ways in which the COVID-19 virus escapes from responses of various immune systems.

Methods: In this review, an electronic search was performed in databases of PubMed, Science Direct, Scopus and Google Scholar, on the genetic diversity of the SARS-CoV-2 virus and its ways of escaping the immune system were evaluated.
Results: Various mutations have been observed in SARS-CoV-2 virus variants. A number of these mutations can affect viral properties such as infectivity and severity of COVID-19 and are associated with the ability to escape immune responses and can lead to inhibition of innate and acquired immune responses.
 Conclusion: By identifying the types of SARS-CoV-2 virus mutations and their association with immune escape routes, we can hope for effective treatments as well as achieving more effective vaccines.
