Background and Aim: Exercise training is considered the most effective non-pharmacological strategy for the prevention of CVDs. The present study aimed to investigate the effects of 4D Pro suspension training on inflammatory, coagulation, and hematological markers associated with cardiovascular diseases in sedentary obese women. Methods: This study was designed as a randomized clinical trial with a pretest–posttest design. Twenty young obese women voluntarily participated in this study. Participants were randomly assigned to either the control or training group. The exercise group performed 4D Pro suspension training for eight weeks. Blood samples were collected in a fasting state before the start of the intervention and after the last training session to measure coagulation indices, hematological parameters, and inflammatory markers. The data were analyzed using analysis of covariance (ANCOVA) in SPSS software. Results: 4D Pro training significantly decreased body weight, body fat percentage, and BMI by 8%, 7.8%, and 5.9%, respectively, compared to baseline. Furthermore, 4D Pro exercise led to significant reductions in platelet count, fibrinogen, PT, PTT, MCV, and hs-CRP, along with increases in factor VIII and hemoglobin concentration (p ≤ 0.0001). After training, platelet count, fibrinogen, factor VIII, and hs-CRP levels were significantly lower in the training group than in the control group. Additionally, PT (p ≤ 0.02) and PTT (p ≤ 0.003) values were more in the training group. Conclusion: Eight weeks of 4D Pro suspension training resulted in improvements in body composition, reduced systemic inflammation, and modulated coagulation and hematological markers associated with cardiovascular risk in sedentary obese women. These findings suggest that 4D Pro training may serve as an effective non-pharmacological intervention to reduce cardiovascular risk in this population.
