Assessment of Transvaginal Ultrasonography Accuracy in the Diagnosis of Uterine Pathologies in Women with Abnormal Uterine Bleeding

Background & aim: The most common cause of abnormal uterine bleeding in women of childbearing age is structural disorders of the uterus. There are several methods for assessing abnormal uterine bleeding, such as transvaginal and hysteroscopic ultrasound. The aim of this study was to determine the accuracy of transvaginal and hysteroscopic ultrasound in diagnosing uterine lesion in women with uterine fibroids.
Methods: The present cross-sectional study fits into correlation schemes in terms of data collection and analysis. The primary study population was 119 women between 20 to 50 years of age with abnormal uterine bleeding undergoing transvaginal ultrasonography and hysteroscopy for therapeutic and diagnostic purposes, from which 109 patients matched the entrance criteria. Patients with exclusion criteria (obesity, underemployment) Thyroid) were excluded from the study and the diagnostic accuracy of transvaginal ultrasound for the diagnosis of lesions such as uterine polyps, submucosal myomas, normal and endometrial hypertrophy and endometrium without lesions with hysteroscopic findings in the form of positive, positive values, characteristics. The likelihood ratio was compared and evaluated. Data were analyzed using Chi-square and Fisher tests.
Results: Ultrasound and hysteroscopic findings in this study indicated that the most common causes of abnormal uterine bleeding were; uterine polyps and submucosal myomas, endometrial hypertrophy. Sensitivity of transvaginal ultrasound for the above three findings were: 58.9%, 72.2% and 51.7% respectively. The specificity of this method for diagnosing these lesions were; 76.8%, 78.5% and 86.4% respectively.
Conclusion: The final results indicated that the diagnostic accuracy of transvaginal ultrasound was significantly lower than that of hysteroscopy in the diagnosis of uterine polyps, submucosal myomas and endometrial hypertrophy.
