Abstract
Background and purpose: Myocardial Infarction (MI) is the destruction and permanent and irreversible cell death of a myocardium’s part. Physical exercise is reported to reduce myocardial apoptosis but its molecular process has yet remained unclear in response to the type of activity and its intensity. Sports activity is likely  to prevent cell death by affecting the most important factors affecting the process of apoptosis. The aim of this study was to investigate the effect of aerobic exercise intensity on apoptosis in rats with myocardial infarction.
Method: In an experimental study, 18 ten weeks male Wistar rats with myocardial infarction were divided into three groups of High Intensity Interval training (HIIT), Low Intensity Interval Training (LIIT) and control group (without exercise). Bcl-2 and Bax cardiomyocyte concentrations were evaluated as the main markers of apoptosis by qRT-PCR method.
Results: Bcl-2 values in the High Intensity Interval training (HIIT)group were significantly higher than the control group (P = 0.077). In the Low Intensity Interval Training (LIIT) group, (P = 0.017) was significantly higher than the control group. Bax values were significantly higher in the group High Intensity Interval training (HIIT) (P = 0.019) than in the control group while it did not significantly increase in the Low Intensity Interval Training (LIIT) group (p = 0.979). BCL-2 values were higher in LIIT group than in the HIIT group (P = 0.023). Bax values in the High Intensity Interval training (HIIT) group were higher than the Low Intensity Interval Training (LIIT) group (P = 0.019).
Conclusion: The results showed that interval training reduced cardiomyocyte apoptosis after myocardial infarction by expressing the Bcl-2 and  Bax gene and its value depended on the intensity of training.  In addition, Low Intensity Interval Training (LIIT) is more effective than  High Intensity Interval training (HIIT). On this basis, it is recommended to pay attention to training intensity in cardiac rehab.
