چکیده :
مقدمه و هدف: از حساسیت به شیر گاو به عنوان یک عامل ایجاد کننده کولیک شیرخوارگی نام برده شده است. بنابراین تعداد زیادی از پزشکان رژیم غذایی این شیرخواران را به رژیم غذایی عاری از شیر گاو تغییر میدهند. در این مطالعه، نقش حساسیت به شیر گاو در کولیک شیرخوارگی در شیرخوارانی که به طور انحصاری از شیر مادر تغذیه میشوند بررسی شده است.
مواد و روش کار: این مطالعه کارآزمایی بالینی یکسو کور بر روی شیرخواران بین 3 هفته تا 3 ماهه مبتلا به کولیک شیرخوارگی که برای واکسیناسیون یا معاینات دوره ای به مراکز بهداشتی ـ درمانی شهر شیراز در سال 1383 مراجعه کرده بودند انجام گردیده است. پس از تائید کولیک در این شیرخواران پرسشنامه ای برای هر کدام از بیماران تکمیل شد که در آن میزان گریه شیرخوار در شبانه روز، سابقه فامیلی آلرژی و سایر علایم همراه ذکر گردید. به والدین آنها آموزش هایی جهت کاهش نفخ شکم شیرخوار داده شد و سپس تست پوستی پریک با شیر گاو و آزمایش مدفوع (برای دیدن خون مخفی در مدفوع) برای تمام این شیرخواران انجام گردید. سپس شیرخوارانی که از شیرخشک استفاده می کردند از ادامه مطالعه خارج شدند و شیرخوارانی که به صورت انحصاری از شیر مادر تغذیه میشدند یعنی 77 نفر به دو گروه مورد (35 نفر ) و شاهد (42 نفر) تقسیم شدند و 50 نفر شیر خوار سالم بدون کولیک نیز به عنوان گروه کنترل فاقد کولیک تحت بررسی قرار گرفتند. در گروه مورد، به مادران توصیه شد که به مدت 2 هفته شیر گاو و سایر لبنیات را مصرف نکنند. اما در گروه شاهد، تغییری در رژیم غذایی مادران ایجاد نشد. پس از دو هفته شیرخواران مجدداً ویزیت شده و میزان گریه آنها در شبانه روز از والدین سؤال گردید. دادهها با استفاده از آزمون کای دو و نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.
یافته ها : شیرخواران مبتلا به کولیک که مادرانشان به مدت 2 هفته لبنیات مصرف نکردند بهبود قابل توجهی نسبت به گروه شاهد نداشتند. شیوع تست پوستی مثبت در شیرخواران مبتلا به کولیک 6/2 درصد بود. اما شیوع تغذیه با شیر خشک و همچنین خونریزی مخفی در مدفوع، به طور واضحی در شیرخواران مبتلا به کولیک بیش از شیرخواران فاقد کولیک بود.
نتیجه گیری: با توجه به عدم پاسخ شیرخواران مبتلا به کولیک به حذف لبنیات از رژیم غذایی، می توان ادعا کرد که حساسیت به شیر گاو علت مهم و شایعی برای کولیک شیرخوارگی نیست، بنابراین حذف لبنیات از رژیم غذایی مادران شیرده توصیه نمیشود. همچنین نیازی به انجام تست پوستی به عنوان یک بررسی معمول در این شیرخواران وجود ندارد.

ABSTRACT
Introduction & Objective: Cow’s milk allergy has been considered as a cause of infantile colic. Many physicians change the diet of these infants to a cow’s milk free diet. In this study, we evaluated the role of cow’s milk allergy in infantile colic in breast fed infants
Materials & Methods: This single blind clinical trial was done in infants between three weeks and three months old with infantile colic who were referred to a subspecialty clinic in Shiraz-Iran from September 2003 to February 2004. Only exclusively breast fed infants were selected. For other infants who were formula fed, only a questionnaire was filled out, stool exam was performed and they were then excluded from the study. Skin prick tests with cow’s milk and stool exams (for occult blood) were performed for these infants, and the correct technique of breast feeding was taught to their mothers. Then, the infants were divided to case and control groups randomly. In the case group, the mothers were advised not to consume cow’s milk and other dairy products for two weeks. But in the control group, no change in the diet of the mothers was recommended. Furthermore, 50 healthy infants without symptoms of colic were included in the study as the second control group. Their mothers were asked about the type of feeding and stool exams were done for them. Finally, the data were analyzed using chi-square test.
Results: From all 270 examined infants, colic was confirmed in 169 cases. Among them, 153 infants were breast fed exclusively. 114 cases in this group allowed the skin prick test to be done. The test was positive in only 3 infants (2.6%). From these 114 infants, only 77 infants completed the study. 35 infants were in the case group and 42 infants were in the control group. Infants with colic whose mothers did not take dairy products, did not improve significantly in comparison with the control group. Prevalence of formula feeding was significantly higher in colicky infants in comparison with non-colicky infants (24% and 2% respectively). Prevalence of occult blood in the stools of colicky infants was significantly higher in colicky infants than non-colicky infants (24% and zero respectively).
Conclusion: Due to the unresponsiveness of infants to the elimination of cow’s milk, it can be concluded that cow’s milk allergy is not a common cause of infantile colic. Therefore, it is not advised to eliminate the dairy products from the diet of nursing mothers. Also, it is not necessary to perform skin prick tests as a routine test in these infants. The high prevalence of occult blood in the stools of infants suffering from colic is a new finding. Evaluation of its cause will be helpful in determining the etiology of infantile colic..
