fa ارزیابی کیفیت اپلیکیشن‌های سلامت همراه مرتبط با پایبندی دارویی در ایران Evaluation of the Quality of Mobile Health Applications for Medication Adherence in Iran تخصصي Special پژوهشي Research زمینه و هدف: اپلیکیشن‌های سلامت همراه به دلیل دسترسی‌پذیری بالا و نفوذ گسترده تلفن‌های هوشمند، به ابزارهایی کلیدی برای ارتقای پایبندی بیماران به درمان دارویی تبدیل شده‌اند. کیفیت این اپلیکیشن‌ها از نظر طراحی، عملکرد، امنیت داده‌ها و تجربه کاربری تأثیر مستقیمی بر اثربخشی آن‌ها دارد. با وجود افزایش استفاده از این ابزارها در ایران، بررسی‌های اندکی به ارزیابی نظام‌مند کیفیت آن‌ها پرداخته‌اند، لذا هدف از این مطالعه تعیین و ارزیابی کیفیت اپلیکیشن‌های سلامت همراه مرتبط با پایبندی دارویی در ایران بود. روش‌ بررسی: این یک مطالعه توصیفی از نوع کاربردی و مقطعی است که در سال ۱۴۰۱ انجام شد. اپلیکیشن‌ها از پلتفرم کافه‌بازار با کلیدواژه‌های «دارو»، «داروخانه» و «یادآور دارو» شناسایی شدند. ۸ اپلیکیشن واجد شرایط انتخاب گردید، برای ارزیابی، از چک‌لیست محقق‌ساخته و ابزار استاندارد MARS استفاده شد. داده‌ها به وسیله چهار ارزیاب مستقل (دو کارشناس فناوری اطلاعات سلامت، یک داروساز بالینی و یک اپیدمیولوژیست) جمع‌آوری گردید. داده‌های جمع‌آوری شده با استفاده از آزمون‌ آماری ضریب کاپای کوهن تجزیه و تحلیل شدند. یافته‌ها: از میان ۱۳۳۶ اپلیکیشن اولیه، ۸ اپلیکیشن واجد شرایط ارزیابی شدند. بیش از نیمی از اپلیکیشن‌ها به‌روز بودند و همگی دارای قابلیت‌هایی مانند امنیت داده‌ها، پشتیبانی از چند کاربر و یادآوری آفلاین بودند. در مقابل، ویژگی‌هایی هم‌چون حریم خصوصی، امتیاز پایبندی و گزارش‌های آماری پایبندی در هیچ‌یک از اپلیکیشن‌ها مشاهده نشد. در مجموع، سطح کیفیت اپلیکیشن‌ها ناهمگون بود و تنها تعداد محدودی از آن‌ها بخش عمده‌ای از ویژگی‌های عملکردی و عملیاتی مورد انتظار را دارا بودند. نتیجه‌گیری: نتایج نشان داد که اپلیکیشن‌های موجود در ایران اگرچه برخی قابلیت‌های پایه‌ای را دارا هستند، اما از نظر برخورداری از ویژگی‌های کلیدی و انطباق با استانداردهای بین‌المللی محدودیت دارند. نبود حریم خصوصی و بازخورد آماری پایبندی از مهم‌ترین کاستی‌ها محسوب می‌شود.   Background & aim: Mobile health(mHealth) applications have emerged as pivotal tools for enhancing medication adherence, leveraging high accessibility and the widespread penetration of smartphones. The clinical effectiveness of these tools is directly influenced by their quality in terms of design, functionality, data security, and user experience. Despite the growing adoption of mHealth in Iran, systematic evaluations of these applications remain scarce. This study aimed to identify and assess the quality of medication adherence applications available in the Iranian market. Methods: This was a descriptive, cross-sectional, and applied study conducted in 2022. Applications were identified through the Cafe Bazaar platform using the keywords "drug," "pharmacy," and "medication reminder." Following a screening process, eight eligible applications were selected for detailed evaluation. Data were collected using a researcher-developed functional checklist and the standardized Mobile Application Rating Scale (MARS). Assessment was performed by four independent raters (two health information technology specialists, one clinical pharmacist, and one epidemiologist). Inter-rater reliability was analyzed using Cohen’s kappa coefficient. Results: From an initial pool of 1,336 applications, eight met the inclusion criteria. While more than half of the evaluated apps were up-to-date and all featured basic data security, multi-user support, and offline reminders, significant gaps were identified. Critical features—including robust privacy protection policies, adherence scoring systems, and comprehensive statistical adherence reports—were absent across all evaluated applications. Overall, the quality of the applications was heterogeneous, with only a limited few possessing the majority of expected functional and operational capabilities. Conclusion: Our findings indicate that while mHealth applications for medication adherence in Iran provide basic functionalities, they fall significantly short of international standards and lack critical features. The absence of privacy safeguards and longitudinal adherence feedback are major deficiencies.
