fa بررسی تأثیر یافته‌های آزمایشگاهی بیماران مبتلا به کووید 19 با تست PCR مثبت در تشخیص، تعیین ،پیش‌آگهی و اپیدمی یا پاندمی‌های آینده کووید 19 در بیمارستان معین دانشگاه علوم پزشکی یاسوج Evaluation of Effectiveness of Para-clinical Findings of Patients with COVID-19 in the Diagnosis, Detection, Prognosis and Future Probably Epidemic or Pandemic of COVID-19 تخصصي Special پژوهشي Research <div class="WordSection1">زمینه و هدف: شدت بیماری کووید 19 و مرگ و میر بالای آن، این بیماری را در دنیا به یک شرایط بحرانی تهدید کننده سلامت تبدیل کرده است، درمان قطعی برای آن وجود ندارد و اکثر درمان‌ها حمایتی است. لذا هدف از این مطالعه تعیین و بررسی تأثیر یافته‌های آزمایشگاهی بیماران مبتلا به کووید 19 با تست PCR مثبت در تشخیص، تعیین ،پیش‌آگهی و اپیدمی یا پاندمی‌های آینده کووید 19 در بیمارستان معین دانشگاه علوم پزشکی یاسوج بود. روش بررسی: در این مطالعه توصیفی که به صورت مقطعی و در مدت زمان 6 ماه در سال 1400-1399انجام شد، تعداد 499 بیمار وارد مطالعه شدند. اطلاعات مورد نیاز از سیستم ثبتی HIS و پرونده‌های بیماران که به دنبال تست PCR مثبت در بیمارستان معین کووید 19 دانشگاه علوم پزشکی یاسوج بستری شده بودند جمع‌آوری شد. داده‌های جمع‌آوری شده با استفاده از آزمون‌های تی دو، رگرسیون تجزیه و تحلیل شدند. یافته‌ها: نتایج نشان داد تعداد 244 نفر(9/48 درصد) مرد و 255 نفر(1/51 درصد) زن بودند. میانگین و انحراف معیار سنی افراد 13/20 ± 77/50 سال بوده است. هم‌چنین اکثریت شرکت کنندگان(2/80 درصد) سیگاری نبودند. سرفه، کاهش حس بویایی، کاهش حس چشایی، تنگی نفس، تب و لتارژی به ترتیب بیشترین فراوانی را در بین علایم بالینی داشتند. هم‌چنین درجه حرارت، تعداد تنفس، تعداد نبض، فشارخون و اشباع اکسیژن خون شریانی دچار تغییراتی از میزان استاندارد به سمت بالا یا پایین بودند. در یافته‌های آزمایشگاهی نیز گلبول‌های سفید، همگلوبین، پلاکت، لنفوسیت، نوتروفیل، سرعت رسوب گلبول های قرمز، CRP(پروتئین های فاز حاد) و نیتروژن اوره خون نیز نسبت به میزان استاندارد تغییراتی داشتند که رابطه این تغییرات با مرگ بیمار سنجیده شد. بر این اساس به ازای یک سال افزایش سن، 5 درصد مرگ بیماران افزایش پیدا می‌کند. هم‌چنین به ازای هر واحد افزایش متغیر لنفوسیت به میزان 7 درصد زنده ماندن افزایش پیدا می‌کند.</div>   <div class="WordSection2"> </div> <div aria-label="شکستن صفحه" class="cke_pagebreak" contenteditable="false" data-cke-display-name="pagebreak" data-cke-pagebreak="1" style="page-break-after:always" title="شکستن صفحه"></div>نتیجه‌گیری: نتایج مطالعه حاضر اطلاعات مفیدی را جهت تیم سلامت در تعیین، تشخیص و درمان در بیماران با تشخیص کووید 19 و جلوگیری از عوارض درمان و بیماری بر بیماران در اپیدمی‌ها و پاندمی‌های احتمالی آینده فراهم می‌کند.   Background & aim: The severity of COVID-19 and its high mortality rate have made this disease a critical health-threatening condition in the world, there is no definitive treatment for it, and most treatments are supportive. Therefore, the aim of the present study was to determine and investigate the impact of laboratory findings of patients with COVID-19 with a positive PCR test on the diagnosis, determination, prognosis, and future epidemics or pandemics of COVID-19 in the appointed hospital of Yasuj University of Medical Sciences. Methods: In the present descriptive cross-sectional study conducted over a period of six months in 2020-2021, the required data on a total of 499 patients was gleaned from the HIS registration system and patients’ medical files who were hospitalized in the designated COVID-19 hospital of Yasuj University of Medical Sciences following a positive PCR test in 2019. The obtained information was analyzed with SPSS using descriptive statistics and inferential statistical tests at a significance level of p<0.01. <a name="_Hlk224342123">Results</a>: Of the total 499 patients whose data were collected, 244 (48.9%) were male and 255 (51.1%) were female. The mean±SD of the age of the subjects was 50.7± 20.13 years. Besides, the majority of the participants (80.2%) were non-smokers. Cough, decreased olfaction, diminished gustation, dyspnea, fever, and lethargy were the most frequent clinical symptoms, respectively. Moreover, body temperature, respiratory rate, pulse rate, blood pressure, and arterial blood oxygen saturation changed from the standard values upward or downward. In laboratory findings, leucocytes, hemoglobin, platelets, lymphocytes, neutrophils, ESR, CRP, and BUN also had changes compared to standard levels, and the relationship of these changes with patient mortality was measured. Accordingly, the mortality rate of patients increased by 5% for every one year of age increase. Further, for every unit increase in lymphocyte variable, the survival rate increased by 7%.
