fa اورژانس زنان در بارداری، موردی نادر از پیچ خوردگی تخمدان به دلیل یک توده ی آدنکسال بزرگ در یک زن باردار ۱۶ هفته Obstetric Emergency in Pregnancy: A Rare Case of Ovarian Torsion Caused by a Large Adnexal Mass in a 16-Week Pregnant Woman بالینی-زنان و زایمان Gynaecology گزارش مورد case report زمینه و هدف: اخیراً، بروز توده‌های آدنکسال در دوران بارداری افزایش یافته است، که دلیل آن  تأخیر در سن بارداری و پیشرفت‌های تصویربرداری یا استفاده‌های متداول‌تر از سونوگرافی بازمی‌گردد. به عنوان یک اثر جانبی، پیچ خوردگی تخمدان می‌تواند در کمتر از یک درصد بارداری‌ها رخ دهد و در سمت راست شایع تر می‌باشد. هدف این مطالعه ارائه یک مورد نادر از پیچ‌خوردگی تخمدان چپ به دلیل توده‌ی آدنکسال بزرگ در یک زن باردار ۱۶ هفته بود. گزارش مورد: بیمار خانم 32 ساله با سومین بارداری در هفته ۱۶، با درد شدید و متناوب شکمی در ناحیه پایین سمت چپ به زایشگاه بیمارستان امام سجاد یاسوج مراجعه کرد. سونوگرافی نشان‌دهنده یک کیست تخمدان چپ به ابعاد ۱۵×۱۲ سانتی‌متر بود. به دلیل افزایش درد و عدم پاسخ به مدیریت محافظه‌کارانه، بیمار تحت لاپاراتومی قرار گرفت. پیچ سه‌بار تخمدان باز شد و کیست برداشت شد. بیمار با موفقیت بهبود یافت و وضعیت جنین تا زمان ترم بدون عارضه بود. نتیجه‌گیری: پیچ‌خوردگی تخمدان در بارداری نادر است، اما در حضور توده‌های بزرگ ادنکسی باید سریع تشخیص داده و درمان شود تا عملکرد تخمدان حفظ و بارداری ایمن باقی بماند. Background & aim: Recently, the incidence of adnexal masses during pregnancy has increased, possibly due to delayed gestational age and advances in imaging or more frequent use of ultrasound. As a side effect, ovarian torsion can occur in less than 1% of pregnancies and is more common on the right side. The aim of the present study was to present a rare case of left ovarian torsion due to a large adnexal mass in a woman at 16 weeks of gestation. Case report: A 32-year-old woman in her third pregnancy at 16 weeks of gestation presented to the obstetric emergency department of Imam Sajjad Hospital if Yasuj, Iran, with severe intermittent lower left abdominal pain. Ultrasonography revealed a large left ovarian cyst measuring 15 × 12 cm. Due to worsening pain and lack of response to conservative management, the patient underwent laparotomy. The ovary was found to be twisted three times; detorsion was performed, and the cyst was excised. The postoperative course was uneventful, and the pregnancy continued to term without complications.
