fa بررسی اثر داروی نیکوراندیل قبل از عمل برروی صدمه به میوکارد پس از عمل جراحی قلب باز Studying the Effect of Preoperative Nicorandil on Myocardial Injury After Open Heart Surgery تخصصي Special پژوهشي Research زمینه و هدف: محافظت ناکافی از میوکارد و آسیب میوکارد حین عمل(PMI) یک عارضه مهم و شایع در روش‌های مختلف جراحی قلب است، لذا هدف از این مطالعه تعیین و بررسی اثر داروی نیکوراندیل قبل از عمل بر روی صدمه به میوکارد پس از عمل جراحی قلب باز بود. روش بررسی: این یک مطالعه تحلیلی از نوع کارآزمایی بالینی یک سوکور می‌باشد که در سال 1402 در مرکز آموزشی درمانی شهید چمران‌ ـ  اصفهان انجام شد. بیماران مورد مطالعه 70 بیمار(هر گروه 35 نفر)بودند که به صورت تصادفی در دو گروه کنترل و مداخله تحت عمل جراحی قلب قرار گرفتند. بر اساس تقسیم‌بندی بر اساس تصادفی‌سازی کامپیوتری در دو گروه مداخله و کنترل تقسیم شدند. در گروه مداخله علاوه بر درمان‌های معمول، قرص نیکوراندیل خوراکی با دوز 10 میلی‌گرم 3 بار تجویز شد. بیماران به روش مشابه بیهوش و به وسیله یک تیم جراحی شدند. پس از عمل جراحی بیماران از نظر معیارهای بالینی شامل، تروپونین هشت وبیست چهار ساعت بعد از عمل، آناتومی عروق کرونر بیماراز نظر تعداد گرفت، مدت زمان بستری در ICU، زمان نیاز به تهویه مکانیکی(ونتیلاتور)، بروز انفارکتوس میوکارد، بروز آریتمی جدید، نیاز به استفاده از بالون پمپ داخل آئورتی(IABP) و موارد مرگ و میر در دو گروه بررسی و مقایسه شدند. داده‌های جمع‌آوری شده با استفاده از آزمون‌های آماری تی، مجذور کای، ویلکاکسون و من ویتنی تجزیه و تحلیل شدند. یافته‌ها: نتایج نشان داد در مطالعه حاضر در گروه کنترل 4/11 درصد مؤنث و 6/88 درصد مذکر با میانگین سنی46/8±42/58  سال و در گروه مداخله 9/42 درصد مؤنث و 1/57 درصد مذکر با میانگین سنی88/6±31/59 سال بوده‌اند. هم‌چنین 50 درصد از افراد شرکت کننده در مطالعه در رنج سنی70ـ60 سال بودند. میانگین EF در گروه کنترل بیشتر43/8±42/54 بود. در 8 ساعت و 24 ساعت بعد از عمل تروپونین در گروه مداخله (8/0±36/1 و 83/0±56/1) به طور معنی‌داری بیش از گروه کنترل (75/0±06/2 و 91/0±44/2) بوده است(05/0p<). نتیجه‌گیری: نتایج مطالعه حاضر نشان داده شده است که استفاده از NICD باعث محافظت از میوکارد در بیماران تحت عمل جراحی بای‌پس قلب می‌شود. علاوه بر این، تجویز NICD پتانسیل کاهش بروز آریتمی پس از عمل را در پی داشت، اما نتایج معنی‌‌دار نبود. هم‌چنین میانگین میزان افزایش تروپونین در گروه مداخله پس از جراحی قلب باز نسبت به میانگین افزایش تروپونین در گروه کنترل کمتر بود.   Background & aim: Inadequate myocardial protection and intraoperative myocardial injury (PMI) are important and common complications in various cardiac surgical procedures. Therefore, the aim of the present study was to determine and investigate the effect of preoperative nicorandil on myocardial injury after open heart surgery. Methods: The present single-blinded clinical trial analysis study was conducted in 2023 at Shahid Chamran Educational and Medical Center - Isfahan. The participants were 70 patients (35 in each group) who were randomly assigned to two control and intervention groups for cardiac surgery. Based on the computer-based randomization, they were divided into two intervention and control groups. In the intervention group, in addition to the usual treatments, oral nicorandil tablets were administered at a dose of 10 mg 3 times. The patients were anesthetized in a similar way and operated by a team. After surgery, the patients were evaluated and compared in terms of clinical criteria including troponin eight and twenty-four hours after surgery, the anatomy of the patient's coronary arteries after counting, the duration of hospitalization in the ICU, the time required for mechanical ventilation (ventilator), the occurrence of myocardial infarction, the occurrence of new arrhythmia, the need for the use of intra-aortic balloon pump (IABP), and mortality in the two groups. The collected data were analyzed using t-test, chi-square, Wilcoxon, and Mann-Whitney statistical tests. Results: The results indicated that in the control group, 11.4% were female and 88.6% were male, with an average age 58.42±8.46 years, and in the intervention group, 42.9% were female and 57.1% were male with The average age 59.31± 6.88 years. Also, 50% of the people participating in the study were in the age range of 60-70 years). Respectively the average EF in the control group was 54.42±8.43. At 8 hours and 24 hours after the operation, troponin in the intervention group (1.36±0.8 and 1.56±0.83) was significantly higher than the control group (2.06±0.75 and 2.44±0.91 (p<0.05. Conclusion: The results of the present study indicated that the use of NICD protected the myocardium in patients undergoing heart bypass surgery. In addition, NICD administration had the potential to reduce the incidence of postoperative arrhythmia, but the results were not significant.
