fa بروز و تظاهرات بالینی آبسه ساب پریوستال تأخیری پس از جراحی دندان‌های مولر سوم فک پایین: یک مطالعه کوتاه‌مدت آینده‌نگر Incidence and Clinical Manifestations of Delayed Subperiosteal Abscess Following Mandibular Third Molar Surgery: A Short-Term follow up Study دندانپزشکی Dentistry پژوهشي Research زمینه و هدف: یکی از عوارض جراحی مولر سوم نهفته و نیمه نهفته مندیبل آبسه  ساب پریوستئال است که تحقیق کمی در این خصوص انجام شده است. لذا هدف از این مطالعه بررسی میزان بروز و تظاهرات کلینیکی آبسه‌‌های ساب‌پریوستئال بعد از جراحی مولر سوم نهفته و نیمه نهفته مندیبل بود. روش بررسی: این مطالعه مشاهده‌ای در سال 1401-1400 به وسیله جراح متخصص فک و صورت در بخش جراحی فک و صورت دانشکده دندان پزشکی قم یا بخش خصوصی انجام شد. جامعه آماری این پژوهش را افرادی که برای جراحی دندان مولر سوم نهفته و نیمه نهفته مندیبل مراجعه کردند، تشکیل داد و شامل 100 نفرشد. جراحی به شیوه معمول و استاندارد صورت گرفت و داده‌ها با استفاده از چک لیست جمع‌آوری شد. داده‌های جمع‌آوری شده با استفاده از نرم‌افزار  SPSS و آزمون‌های آماری تست دقیق فیشر و کای اسکوئر تجزیه و تحلیل شدند. یافته‌ها: این مطالعه بر روی 100 بیمار با میانگین سنی 27/26 سال انجام شد.70 درصد نمونه‌ها زن و30 درصد آنها مرد بودند. 21 درصد دندان‌ها در موقعیت کلاسI ، 76 درصد در موقعیت کلاسII ، 3 درصد در موقعیت کلاسIII، 2 درصد دندان‌ها کلاسA ، 72 درصد سطح B و 26 درصد سطحC  بر اساس طبقه بندی پل و گریگوری بودند بودند. میزان بروز این عارضه 4 درصد به دست آمد که در دندان‌های مولرسطح c بود(05/0p<). بروز 2 درصد از آبسه‌ها در دندان‌های کلاس II و 2 درصد دیگر در دندان‌های کلاس III <a name="_Hlk132117268">P&G</a> دیده شد(05/0p<). ارتباط آماری بین سن، جنس و جهت‌گیری مولر سوم با میزان بروز آبسه مشاهده نشد(05/0p<). نتیجه‌گیری: آبسه ساب پریوستئال عارضه‌ای با شیوع 4 درصد است که امکان بروز همراه با تظاهرات کلینیکی مانند درد، تورم تا دو دندان مجاور مولر سوم، دو هفته یا بیشتر بعد از جراحی را دارد. موقعیت قرارگیری مولرسوم تحتانی نسبت به راموس و عمق نهفتگی استخوانی از عوامل خطر این عارضه است. امید است که این پژوهش زمینه‌ای باشد تا افراد ذی‌ربط پژوهش‌های جامع‌تری را در خصوص این موضوع با اهمیت انجام دهند.   Abstract Background & aim: One of the complications of mandibular impacted and semi-impacted third molar surgery, is subperiosteal abscess (SPA) that few studies have been conducted. Therefore, the present study was conducted with the aim of investigating the incidence and clinical features of these abscesses. Methods: The present observational study was conducted from 2020 to 2021 by a maxillofacial surgeon at the maxillofacial surgery department of Qom Dental School or the private sector. The statistical population of the present study consisted of patients who were referred for surgery of impacted and semi-impacted third molars of the mandible and included 100 people. The surgery was performed in a standard and routine manner and the data were collected using a checklist. The collected data were analyzed using the Prescriber software and version 22 and SPSS statistical software and the Fisher's exact test and Chi-square statistical tests. Results: The current study was conducted on 100 patients with an average age of 26.27 years. 70% of the subjects were female, and 30% were male. 21% of teeth in class I position, 76% in class II and 3% in class III. 2% of teeth were level A, 72% level B and 26% level C. The incidence rate of this complication was 4%, which was observed in molar teeth in level C P&G (P<0/05). 2% of abscesses were observed in teeth with class II and another 2% in teeth with class III P&G Classification (P<0/05). Statistically significant relationship between age, gender and orientation the third molar and the incidence of abscesses were not observed(P>0/05) Conclusion: Subperiosteal abscess is a complication with a prevalence of 4%, which can occur with clinical manifestations such as pain, swelling up to two teeth adjacent to the third molar two weeks or more after surgery. The position of the lower molar in relation to the ramus and depth of bone embedment are risk factors for this complication.
