fa شناسایی شاخص‌های مرتبط با مدیریت همه‌گیری‌های تنفسی به شیوه دلفی فازD و مدل سازی ساختاری تفسیری Evaluation the Strategies for Explaining the Managed Care Model for Emerging Respiratory Infectious Diseases(COVID-19) in Iran's Health System مدیریت بهداشتی Health Management پژوهشي Research زمینه و هدف: بشر، طی دهه‌های اخیر، از طرفی با پدیده‌های جوی و تغییرات محیطی و از طرف دیگر، با ظهور بیماری‌های نوپدید مواجه بوده است. مدیریت بیماری‌های نوپدید، مستلزم درنظرگرفتن رویکرد اخلاقی برای کنترل و مراقبت از این بیماری‌هاست. معضلات اخلاقی حاصل از پیشرفت‌های تشخیصی و درمانی نوین در علم پزشکی، ضرورت پرداختن به اخلاق پزشکی را دوچندان کرده است. یکی از بیماری‌های نوپدید در قرن حاضر که تقریباً، همه جهان با آن روبه‌رو شده‌اند، بیماری کووید 19است. لذا هدف از این مطالعه تعیین و شناسایی شاخص‌های مرتبط با مدیریت همه‌گیری‌های تنفسی به شیوه دلفی فاز D و مدلسازی ساختاری تفسیری بود. روش بررسی: این پژوهش، یک مطالعه موردی و مصاحبه نیمه ساختارمند می‌باشد که در سال 1400 بر روی 12 نفر از صاحب‌نظران و استادان انجام و منجر به تدوین معیارهای مدل تحقیق شد. برای دستیابی به این هدف ابتدا عوامل تأثیرگذار و شاخص‌های مؤثر بر الگوی مراقبت‌های مدیریت شده برای بیماری‌های واگیر دار تنفسی نوظهور بر اساس تکنیک تحلیل محتوا مشخص و سپس این عوامل و شاخص‌ها دسته‌بندی‌ شدند. داده‌های این پژوهش به روش تحلیل مضمون با کمک نرم‌افزار MAXQDA تحلیل شد و 12 نفر از میان افراد مؤثر در مدیریت این بیماری در داده‌هـای جمع‌آوری شده با استفاده از آزمـون‌هـای آمـاری مـدل‌هـا ی خطی تعمیم یافته، تی هتلینگ و پـس آزمـون بـون فرونـی تجزیه و تحلیل شدند. یافته‌ها: در نتایج این پژوهش 5 مقوله اصلی شناسایی شدند. مقوله‌های اصلی شامل؛ تأمین مالی، تأمین منابع، سرویس‌دهی و مجهزسازی، اطلاعات و پژوهش‌ها و عوامل دولتی بودند. با توجه به خروجی محاسبات ISM متغیر اطلاعات و تحقیقات(C4) و عوامل دولتی(C5) سطح اول است. برای تعیین عناصر سطح دوم، متغیرهای سطح اول حذف شده و یک‌بار دیگر مجموعه ورودی‌ها و خروجی‌ها بدون در نظر گرفتن متغیرهای سطح اول محاسبه شدند. براساس مجموعه مشترک شناسائی و متغیرهایی که اشتراک آنها برابر مجموعه ورودی‌ها باشد، متغیرهای سطح دوم انتخاب شدند. با توجه به خروجی محاسبات ISM متغیرهای تأمین مالی(C1) و تأمین منابع(C2)سطح دوم هستند. نتیجه‌گیری: مراکز درمانی می‌توانند با افزودن ظرفیت‌ها و استانداردها، آمادگی بالایی برای مقابله با بحران داشته باشد. با توجه به این که این بیماری‌های واگیردار تنفسی نوظهور تهدیدی برای کل مردم جهان شده است، نیازمند سیاست‌ها و اقدامات به موقع، مناسب و مقرون به صرفه برای مهار و کاهش پیامدهای مرگ‌بار آن است. برنامه‌ریزی مدیریت بحران ویروس کرونا، نیازمند استراتژی، سیاست گذاری و اقدام صحیح است.   Background & aim: People, during recent decades, has been faced with atmospheric phenomena and environmental changes on the one hand, and with the emergence of new diseases on the other hand. The management of emerging diseases requires considering the ethical approach to control and care for these diseases. Ethical dilemmas resulting from new diagnostic and therapeutic advances in medical science have folded the necessity of dealing with medical ethics. One of the emerging diseases in the present century that practically the world has faced is the COVID-19 disease. Therefore, the purpose of the present study was to determine and identify the indicators related to the management of respiratory epidemics using Delphi phase D method and interpretive structural modeling. Methods: The present case study and semi-structured interview was conducted on 12 experts and professors in 2021 and led to the formulation of research model criteria. In order to achieve this goal, the influencing factors and effective indicators on the model of managed care for emerging respiratory infectious diseases were identified based on the content analysis technique. At that point, these factors and indicators were categorized. The collected data were analyzed by thematic analysis with the help of MAXQDA. Twelve people among the effective people in the management of this disease were analyzed in the collected data using statistical tests of generalized linear models, Hetling's t-test and Bon Feroni's post-test. Results: The results of the present research identified five main categories which were: financial provision, provision of resources, servicing and equipping, information and research and government agents. According to the output of ISM calculations, the variable of information and research (C4) and government factors (C5) were the first level. To determine the elements of the second level, the variables of the first level were removed and once again the set of inputs and outputs were calculated without considering the variables of the first level. Based on the common set of identification and variables whose commonality was equal to the set of inputs, they were selected as second level variables. According to the output of ISM calculations, the variables of financial provision (C1) and resource provision (C2) were the second level. Conclusion: Medical centers can be highly prepared to deal with the crisis by adding capacities and standards. Due to the fact that these emerging respiratory infectious diseases have become a threat to the entire population of the world, it requires timely, appropriate and cost-effective policies and measures to control and reduce its deadly consequences.
