fa بررسی شیوع پوسیدگی‌های زودرس دوران کودکی و برخی عوامل مرتبط با آن، در کودکان 6 -3 سال استان کهگیلویه و بویراحمد در سال 1398 Investigating the Prevalence of Early Childhood Caries and its Related Factors in Children aged 3-6 Years in Kohgiluyeh and Boyer-Ahmad Province in 2019 دندانپزشکی Dentistry پژوهشي Research زمینه و هدف: پوسیدگی زودرس دندانی(Early Childhood Caries) یکی از مشکلات شایع بهداشتی است و فاکتورهای متعددی در شروع و پیشرفت آن نقش دارند که با بررسی آن‌ها و آموزش به موقع والدین، می‌توان نقش مهمی در کنترل این بیماری ایفا کرد. هدف از این مطالعه تعیین و بررسی شیوع پوسیدگی‌های زودرس دوران کودکی و برخی عوامل مرتبط با آن، در کودکان 6 ـ3 سال استان کهگیلویه و بویراحمد در سال 1398 بود. روش بررسی: این تحقیق یک مطالعه توصیفی ـ تحلیلی از نوع مقطعی بود و 565 کودک 3 تا  6 ساله <a name="_Hlk119429086">مهدکودک‌ها و مراکز پیش‌دبستانی شهرهای یاسوج، دهدشت و گچساران با روش نمونه‌گیری تصادفی خوشه‌ای چند مرحله‌ای  انتخاب و وارد مطالعه شدند. </a>تشخیص پوسیدگی وثیت آن به وسیله 3 نفر دانشجوی دندانپزشکی(هر شهر یک دندانپزشک )انجام شد. جمع‌آوری داده‌های دموگرافیک و عوامل مرتبط با پوسیدگی زود رس دندانی با استفاده از چک لیست انجام شد. داده‌های جمع‌آوری شده با استفاده از آزمون‌های آماری کای اسکوئر و رگوسیون لجستیک تجزیه و تحلیل شدند. یافته‌ها: میانگین و انحراف  معیار سنی کودکان 94/0+03/5  سال بود. 306 کودک پسر(2/54 درصد) و 259(8/45 درصد) دختر بودند. تقریباً 75 درصد از کودکان پوسیدگی زودرس دندانی داشتند و میانگین  69/3 ± 13/5 بود. بین پوسیدگی  زودرس  دندانی و  وضعیت اشتغال مادران(0001/0p<)، وضعیت اشتغال پدران(0001/0p<)، زمان شروع مسواک زدن  برای کودک(001/0=p)، انجام  یا عدم  انجام  فلورایدتراپی(0001/0=p)، تعداد دفعات تغذیه باشیرینی جات در بین وعده‌های  غذایی(04/0=p)، زمان  اولین  ویزیت  دندانپزشکی(002/0=p) و تحصیلات مادران وجود داشت(01/0=p) ارتباط معنی‌دار آماری وجود داشت. بر اساس رگرسیون لجستیک، سن پدر(003/0=p، 24/1ـ04/1=OR ، CI95% 12/1=OR)، وضعیت انجام فلورایدتراپی(0001/0=p، 18/9ـ97/1=OR ، CI95% 26/4=OR) و شاخص. شاخص dmft(dدندانهای پوسیده،mپرشده وf هز دست رفته) (0001/0=p، 4/2ـ76/1=OR، CI95% 05/2=OR ) پیش‌ بینی کننده‌های  پوسیدگی زودرس دندانی بودند. <a>نتیجه</a><a class="msocomanchor" href="#_msocom_1" id="_anchor_1" language="JavaScript" name="_msoanchor_1" on<illegal tag>mouseout="msoCommentHide('_com_1')" on<illegal tag>mouseover="msoCommentShow('_anchor_1','_com_1')">[n1]</a> ‌گیری: با توجه به میزان شیوع بالای پوسیدگی‌های زودرس دوران کودکی در کودکان 6ـ3 سال، انجام مداخلات آموزشی به منظور افزایش آگاهی والدین در خصوص نحوه پیشگیری از پوسیدگی‌های زودرس دوران کودکی شامل استفاده ازوارنیش فلوراید، خمیردندان حاوی فلوراید،نحوه صحیح مسواک زدن تحت نظارت والدین، مشاوره رژیم غذایی،شیر دادن به کودک به طور مناسب و خوداری از بیش از حد شیر دادن به کودک در طول شب و اولین ملاقات دندان پزشکی کودک ضروری می‌باشد.   <div> <hr align="left" class="msocomoff" size="1" width="33%" > <div> <div class="msocomtxt" id="_com_1" language="JavaScript" on<illegal tag>mouseout="msoCommentHide('_com_1')" on<illegal tag>mouseover="msoCommentShow('_anchor_1','_com_1')"><a name="_msocom_1"></a> <a class="msocomoff" href="#_msoanchor_1">[n1]</a></div> </div> </div> Background & aim: Early childhood caries is one of the most common health problems, and numerous factors play a role in its onset and progression. Investigating these factors and providing timely training for parents can play an important role in controlling the disease. The present study aimed at determining the prevalence of early childhood caries and its related factors in children aged 3-6 years in Kohgiluyeh and Boyer-Ahmad province in 2019. Methods: In the present cross-sectional descriptive-analytical, 565 children aged 3 to 6 years from kindergartens and preschool centers in the cities of Yasuj, Dehdasht and Gachsaran located at Kohgiluyeh and Boyer-Ahmad province, Iran, were selected and included in the study using a multi-stage cluster random sampling method. The caries diagnosis was done by 3 dental students (one dentist in each city). Collecting demographic data and factors related to premature dental caries was done using a checklist. The collected data were analyzed using Chi-square statistical tests and logistic regression. Results: Mean age of children was 5.03± 0.94.  Out of 565 children, 306(54.2%) were boy and 259(45.8%) were girl. 75% of children had early childhood dental caries. Mean of dmft was 5.13 ± 3.69. There was a statistically significant relationship between early childhood dental caries and employment status of mothers (p<0.0001), and fathers (p<0.0001). Early childhood dental caries was less frequent in children whose mothers were housewives and more frequent in children with unemployed fathers. There was correspondingly a significant relationship between early childhood dental caries and the onset of brushing in a child (p = 0.001), doing or not failing to do fluoride therapy (p = 0.0001), frequency of feeding with sweets between meals (p = 0.04), the time of the first dental visit (p = 0.002), and mothers’ education (p = 0.01). In logistic regression analysis, it was indicated that the father’s age (OR = 1.12, CI95% OR = 1.04-1.24, p=0.003), fluoride therapy status (OR = 4.26, CI95% OR = 1.97-9.18, p=0.0001), and dmft (OR = 2.05, CI95% OR = 1.76-2.4, p=0.0001) were predictors of early childhood dental caries.
