fa ارتباط بین پروتئین واکنشگر C و لنفوپنی با شدت بیماری در بیماران بستری مبتلا به کووید 19 شهر یاسوج در سال 1399 The Relationship Between C-reactive Protein (CRP) and Lymphopenia with Disease Severity in Hospitalized Patients with Covid-19 in Yasuj, Iran in 2020 بالینی-داخلی Internist پژوهشي Research زمینه و هدف: کووید 19 ویروس نوظهوری است که تا کنون میلیون‌ها نفر را مبتلا کرده و سبب ایجاد یک وضعیت بحرانی در سراسر جهان گردیده است، به دلیل شیوع بالای بیماری کوویدـ19 و ظرفیت محدود درمانی، شناسایی سریع فاکتورهای نشان دهنده‌ شدت بیماری، می‌تواند بسیار کمک کننده باشد. هدف از این مطالعه تعیین و بررسی ارتباط بین پروتئین واکنشگر C و لنفوپنی با شدت بیماری در بیماران بستری مبتلا به کووید 19 شهر یاسوج در سال 1399 بود. روش بررسی: این یک مطالعه توصیفی از نوع گذشته‌نگر و به روش نمونه‌گیری در دسترس می‌باشد که در سال 1399 بر روی 206 نفر از افراد PCR  مثبت بستری شده در بیمارستان‌های شهید جلیل و امام سجاد (ع) شهر یاسوج انجام شد. میزان اکسیژن خون، نمای سی‌تی و میزان CRP و لنفوسیت خون از فاکتورهای اصلی مورد بررسی در این مطالعه می‌باشند. روش نمونه‌‌گیری به صورت سرشماری و با استفاده از یک چک لیسـت از پـیش تعیین شده بود و اطلاعات مورد نیاز از پرونده‌های بیماران بستری در بیمارستان استخراج گردید. در این مطالعه، اطلاعات زمینهای بیماران شامل سن، جنس علایم بالینی، مـدت زمـان بسـتری در بیمارستان وضعیت بیمار مبنی بر ترخیص با بهبودی نسـبی یـا فوت جمعآوری گردید. هم‌چنین اطلاعات پـاراکلینیکی شـامل؛ درصد اشباع اکسـیژن خـون، تعـداد گلبـول سـفید، لنفوسـیت،CRP و میزان درگیری ریه در سی‌تی اسکن از پرونده بیماران استخراج شد. داده‌های جمع‌آوری شده با استفاده از آزمون‌های ویلیکاسیون وآنالیز واریانس یک راهه تجزیه و تحلیل شدند . یافته‌ها: شرکت کنندگان در پژوهش حاضر شامل 206 نفر بودند که میانگین سنی آنها برابر با 39/51 سال بود. هم‌چنین 103 نفر(50 درصد) از افراد شرکت کننده در پژوهش حاضر مرد و 103 نفر(50 درصد) نیز زن بودند. نتایج تحلیل واریانس یک ‌طرفه نشان داد که میزان لنفوسیت در افراد مبتلای شدید، متوسط و خفیف بر اساس میزان اکسیژن تفاوت معنی‌داری دارد(03/0=p)، اما میزان CRP در این افراد بر اساس میزان اکسیژن خون(32/0=p) و سی‌تی اسکن(07/0=p) تفاوت معنی‌داری نشان نداد. علاوه بر این میزان لنفوسیت در بیماران مبتلا به کرونای شدید، متوسط و خفیف بر اساس سی‌تی اسکن تفاوت معنی‌داری ندارد(14/0=p).  نتیجه‌گیری: با توجه به یافتههای به دست آمده از پژوهش حاضر، به نظر میرسد در پیشبینی شدت بیماری در بیماران مبتلا به کووید 19 میتوان از میزان لنفوپنی استفاده کرد، اما از میزان CRP در پیش بینی شدت بیماری نمی‌توان استفاده کرد. Background & aim: Covid-19 is an emergent virus which has infected millions of people and triggered a serious circumstance all over the world. Due to the high prevalence of Covid-19 and limited treatment capacity, quick identification of factors which indicate the severity of the disease can be very helpful. The purpose of the present study was to determine and investigate the relationship between C-reactive protein and lymphopenia with the severity of the disease in hospitalized patients with Covid-19 in Yasuj, Iran, in 2019. Methods: The present descriptive study was of retrospective type and available sampling method was used in 2019 on 206 patients with covid-19 where blood oxygen level, CT scan and CRP and blood lymphocyte levels were the main factors investigated. The sampling method was census and using a predetermined checklist, the required information was extracted from the files of patients admitted to the hospital. In this study, the background information of the patients including age, gender, clinical symptoms, length of stay in the hospital, the patient's condition based on discharge with relative recovery or death were collected. Furthermore, paraclinical information including: percentage of blood oxygen saturation, white blood cell count, lymphocyte, CRP and lung involvement in CT scan were extracted from the patients' files. The collected data were analyzed using the Willicion test and one-way analysis of variance. Results: The participants in the present study included 206 people whose average age was 51.39 years. 103 (50%) were men and 103 (50%) were women. The results of one-way analysis of variance indicated that there was a significant difference in the amount of lymphocytes in patients with severe, moderate and mild disease based on the oxygen level (p=0.03), but the CRP level in these patients was based on the blood oxygen level (p=0.32).  CT scan (p=0.07) did not show any significant difference. In addition, there was no significant difference in the amount of lymphocytes in patients with severe, moderate and mild corona based on CT scan (p=0.14).
