fa مطالعه نسبت فراوانی لنفوسیت‌های Treg/Th1 و Treg/Th2 در بیماران لنفوم ‌هاچکین و ارتباط آن با پیش‌آگهی بیماری Studying the Frequency Ratio of Treg/Th1 and Treg/Th2 Lymphocytes in Hodgkin's Lymphoma Patients and its Relationship with Disease Prognosis ایمونولوژی Immunology پژوهشي Research زمینه و هدف: سلول‌های T تنظیمی CD4+CD25+FOXP3+ (CD4+CD25+FOXP3+ Tregs)، گروهی از لنفوسیت‌های T هستند که نقش مهمی در تنظیم پاسخ‌های سیستم ایمنی از طریق سرکوب فعالیت سایر سلول‌های T ایفا می‌کنند و باعث برقراری هموستاز در سیستم ایمنی می شوند. سلول‌های Th1 و Th2 گروهی دیگر از سلول‌های CD4+ هستند که نقش مهمی در شکل‌گیری پاسخ‌های التهابی دارند. با توجه به نقش مهم سلول‌های Treg ، Th1 و Th2 در پاسخ‌های ایمنی در بیماران مبتلا به لنفوما، در این مطالعه، فراوانی لنفوسیت‌های Treg/Th1 و Treg/Th2 در بیماران لنفوم ‌هاچکین که در فازهای مختلف بیماری بودند (بیماران جدید، بیماران در فاز بهبود و بیماران در فاز عود) و ارتباط آنها با  پیش آگهی بیماری بررسی شد.. روش بررسی: در این مطالعه توصیفی ـ مقطعی، 47 بیمار لنفوم هاچکین که شامل 10 مورد جدید(new case)، 24 مورد بهبود یافته(remission) و 13 مورد عود کرده(relapsed) بودند وارد مطالعه شدند. با توجه به محدود بودن تعداد بیماران مراجعه کننده مبتلا به لنفوم هوچکین، کلیه بیماران واجد شرایط که در فاصله زمانی طی سال های 1399 تا 1397 به بخش هماتولوژی و انکولوژی درمانگاه شهید مطهری و هم‌چنین بیمارستان امیر مراجعه کرده بودند، وارد مطالعه شدند. فراوانی لنفوسیت‌های Treg،Th1 و Th2 در خون محیطی با کمک روش فلوسیتومتری اندازه‌گیری شد و سپس نسبت جمعیت‌های  Treg/Th1 و Treg/Th2 مشخص گردید. داده‌های جمع‌آوری شده با استفاده از آزمون‌های من ویتنی و کروکسال والیس تجزیه و تحلیل شدند. یافته‌ها: نتایج نشان دادند که نسبت لنفوسیت‌های Treg/Th1 در موارد بهبود یافته(025/0 درصد) و عود کرده (007/0 درصد) در مقایسه با موارد جدید(16/0 درصد)(به ترتیب با 001/0p<* و 001/0p<*) و هم‌چنین موارد عود کرده در مقایسه با موارد بهبود یافته کاهش معنی‌دار ی یافته است(023/0=p*). در مورد نسبت لنفوسیت‌های Treg/Th2 نیز کاهش معنی‌دار آنها در موارد بهبود یافته(43/0 درصد و عود کرده(34/0 درصد) در مقایسه با موارد جدید(27/4 درصد) مشاهده شد(به ترتیب با 001/0p<* و 001/0p<*). هم‌چنین، با در نظر گرفتن پارامتر امتیاز شاخص بین المللی پیش آگهی یا IPI score، نسبت سلول‌های Treg/Th1 در بیماران لنفوم در گروه با ریسک کم ـ متوسط  بیشتر از گروه  با ریسک بالا بود) میانه 031/0 درصد در مقابل 013/0 درصد، 007/0=p*). به علاوه، نسبت سلول‌های Treg/Th1 و Treg/Th2 در بیماران لنفوم ‌هاچکین در گروه با وضعیت پرفورمانس  (PS) کمتر از 2 )(PS<2 بیشتر از گروه  با وضعیت پرفورمانس مساوی یا بیشتر از 2 (PS≥2) بود(به ترتیب میانه 036/0 درصد در مقابل013/0 درصد، 001/0=*p و 78/0 درصد در مقابل 37/0 درصد، 013/0=*p). نتیجه‌گیری: نتایج این مطالعه نشان داد که افزایش نسبت لنفوسیت‌های Treg/Th1 و Treg/Th2 با پاسخ بهتر نسبت به درمان و هم‌چنین پیش‌آگهی بهتر در بیماران لنفوم ‌هاچکین مرتبط می‌باشد.     Background & aim: CD4+CD25+FOXP3+ regulatory T cells (CD4+CD25+FOXP3+ Tregs) are a group of T lymphocytes that play an important role in regulating immune system responses by suppressing the activity of other T cells and establishing homeostasis in the immune system. be. Th1 and Th2 cells are another group of CD4+ cells that play an important role in the formation of inflammatory responses. Considering the important role of Treg, Th1 and Th2 cells in immune responses in patients with lymphoma, the present study investigated the frequency of Treg/Th1 and Treg/Th2 lymphocytes in Hodgkin's lymphoma patients who were in different phases of the disease (new patients, patients in recovery phase and patients in the relapse phase) and their relationship with disease prognosis. Methods: In the present cross-sectional descriptive study, 47 Hodgkin's lymphoma patients including 10 new cases, 24 remission cases and 13 relapsed cases were included in the study. Due to the limited number of patients with Hodgkin's lymphoma, all eligible patients who referred to the hematology and oncology department of Shahid Motahari Clinic and Amir Hospital between 2019 and 2017 were included in the study. The frequency of Treg, Th1 and Th2 lymphocytes in peripheral blood was measured with the help of flow cytometry method and then the ratio of Treg/Th1 and Treg/Th2 populations was determined. Collected data were analyzed using Mann-Whitney and Croxall-Wallis tests. P value less than 0.05 was considered significant. Results: The results indicated that the ratio of Treg/Th1 lymphocytes in recovered cases (0.025%) and relapsed cases (0.007%) compared to new cases (0.16%) (respectively with p<0.001* and p<0.001*) and relapsed cases were significantly reduced compared to recovered cases (p*=0.023). Regarding the ratio of Treg/Th2 lymphocytes, their significant decrease was observed in recovered cases (0.43%) and relapsed (0.34%) compared to new cases (4.27%) (p<0.001, respectively). * and p<0.001*). Also, taking into account the International Prognostic Index (IPI) score parameter, the ratio of Treg/Th1 cells in lymphoma patients in the low-moderate risk group was higher than the high-risk group (median). 0.031% versus 0.013%, p=0.007*). In addition, the ratio of Treg/Th1 and Treg/Th2 cells in Hodgkin's lymphoma patients was higher in the group with performance status (PS) less than 2 (PS<2) than in the group with performance status equal to or greater than 2 (PS≥2). The median of 0.036% versus 0.013%, p=0.001 and 0.78% versus 0.37%, p=0.013) respectively.
