fa تبیین الگوی مراقبت‌های مدیریت شده برای بیماری‌های واگیردار تنفسی نوظهور در نظام بهداشت و درمان ایران در سال Clarifying the Model of Managed Care for Emerging Respiratory Infectious Diseases in Iran's Healthcare System in 2019 مدیریت بهداشتی Health Management پژوهشي Research زمینه و هدف: مراقبت مدیریت شده نمونه‌ای از نظام‌های نوین مراقبت سلامتی است که وظیفه آن کنترل هزینه خدمات، در یک حالت هزینه اثر بخشی مناسب از طریق سازمان‌های مراقبت مدیریت شده می‌باشد. نظر به این که عوامل متعددی چون ناکافی بودن بودجه، انجام نسخه‌نویسی و آزمایش‌های غیرضروری، سیستم نادرست پرداخت در ایران مشاهده می‌شود، لذا هدف از این مطالعه تعیین و تبین الگوی مراقبت‌های مدیریت شده برای بیماری‌های واگیردار تنفسی نوظهور در نظام بهداشت و درمان ایران بود. روش بررسی:  این پژوهش، یک مطالعه موردی و مصاحبه نیمه ساختارمند می‌باشد که در سال 1400 بر روی 12 نفر از صاحب‌نظران و اساتید انجام و منجر به تدوین معیارهای مدل تحقیق منجر شد. برای دستیابی به این هدف ابتدا عوامل تأثیرگذار و شاخص‌های مؤثر بر الگوی مراقبت‌های مدیریت شده برای بیماری‌های واگیر دار تنفسی نوظهور بر اساس تکنیک تحلیل محتوا مشخص و سپس این عوامل و شاخص‌ها دسته‌بندی‌ شدند. با توجه به این که هدف این پژوهش ترکیبی اکتشافی از نوع متوالی و مدل ابزارسازی، طراحی و تدوین الگو بر اساس معیارهای مورد طراحی مدل است. داده‌هـای جمع‌آوری شده با استفاده آزمـون‌هـای آمـاری مـدل‌هـا ی خطی تعمیم یافته، تی هتلینگ و پـس آزمـون بـون فرونـی تجزیه و تحلیل شدند. یافته‌ها: بر اساس مصاحبه‌های انجام ‌شده و بررسی پژوهش‌های پیشین، عوامل مؤثر بر الگوی مراقبت‌های مدیریت شده برای بیماری‌های واگیر دار تنفسی نوظهور شناسایی‌شد و در پنج بعد مجزا تقسم شده و بر اساس کدگذاری محوری در 13 دسته قرار گرفتند. در این مدل 5 مقوله اصلی تأمین مالی، تأمین منابع، اطلاعات و تحقیقات، سرویس دهی و مجهزسازی و هم‌چنین عوامل دولتی به عنون مقوله های اصلی شناسایی شدند. <a name="_Hlk129038791">هم‌چنین </a>وضعیت مراقبت مدیریت شده بیماری واگیردار تنفسی نوظهور( کرونا )در رتبه اول و سایر بیماری‌ها در رتبه‌های بعدی  قرار داشتند. مراقبت مدیریت شده مراکز روستایی به طور معنی‌داری از مراکز شهری وضعیت بهتری داشتند. نتیجه‌گیری: در مجموع باید گفت که برنامه‌ریزی جهت برقراری این سازمان‌ها به عنوان بخش خصوصی بیمه‌ای در کنار بخش درمان امری اجتناب ناپذیر است، لذا الگویی که بتواند نظام سنتی را تسهیل و به سمت نظام نوین سوق یابد الزامی است.   Background & aim: managed care is an example of modern health care systems whose task is to control the cost of services in a cost-effective manner through managed care organizations. Considering that several factors such as inadequate budget, unnecessary prescriptions and tests, incorrect payment system are observed in Iran. Therefore, the purpose of the present study was to determine and explain the model of managed care for emerging respiratory infectious diseases in Iran's healthcare system. <pre> </pre> Methods: The present case study and semi-structured interview was conducted on 12 experts and professors in 2019 leading to the development of research model criteria. For emerging respiratory infectious diseases, based on content analysis technique, these factors and indicators are categorized. Considering that the purpose of this research is an exploratory combination of the sequential type and the tooling model, designing and editing the model based on the criteria of the model design. The collected data were analyzed using statistical tests of generalized linear models, Hotelling's t and Bonferroni's post-test. Results: Based on the conducted interviews and review of previous researches, Factors affecting the managed care pattern for emerging respiratory infectious diseases were identified and divided into five separate dimensions and placed into 13 categories based on axial coding. In this model, 5 main categories of financing, providing resources, information and research, servicing and equipping, as well as government agents were identified as the main categories. Also, the managed care status of emerging respiratory infectious disease (Corona) was ranked first and other diseases were ranked next. The managed care of rural centers was significantly better than urban centers.
