fa بررسی فنوتیپی و ژنوتیپی مقاومت متالوبتالاکتامازی در ایزوله‌های بالینی اشریشیاکلی و کلبسیلاپنومونیه Phenotypic and Genotypic Investigation of Metallobetallactamase Resistance in Clinical Isolates of Escherichia Coli and Klebsiella Pneumoniae میکروبیولوژی Microbiology پژوهشي Research زمینه و هدف: استفاده از سفالوسپورین‌های طیف وسیع در درمان عفونت‌ها، باعث پیدایش دسته جدیدی از بتالاکتامازها به نام متالوبتالاکتاماز گردیده است، این آنزیم‌ها به بتالاکتامازهای کلاس B آمبلر تعلق دارند. متالوبتالاکتامازهای گروه A وB در انتروباکتریاسه‌ها مشکلاتی در درمان ایجاد می‌کنند. این مطالعه با هدف بررسی فنوتیپی سویه‌های مولد متالوبتالاکتاماز اشریشیاکلی و کلبسیلاپنومونیه و هم‌چنین بررسی مولکولی ژن های SPM، VIM، SIM، IMP و GIM انجام شد. روش بررسی: این یک مطالعه توصیفی‌ـ کمی می‌باشد که بر روی 185 بیمار بستری در بخش‌های مراقبت‌های ویژه بیمارستان میلاد تهران در تاریخ اسفندماه تا اردیبهشت ماه 1399ـ1398 انجـــام گرفت و 45 سویه اشریشیاکلی و 62 سویه کلبسیلاپنومونیه انتخاب شدند. جهت بررسی مقاومت آنتی‌بیوتیکی سویه‌ها از آزمون انتشار در آگار و هم‌چنین جهت بررسی فنوتیپی سویه‌های مولد متالوبتالاکتاماز، از روش دیسک ترکیبی استفاده شد. بررسی مولکولی ژن‌های مولد متالوبتالاکتاماز به روش PCR انجام گردید. داده‌های جمع‌آوری شده با استفاده آزمون فیشر تجزیه و تحلیل شدند. یافته‌ها: در مطالعه حاضر بیشترین مقاومت آنتی‌بیوتیکی سویه‌های اشریشیاکلی و کلبسیلاپنومونیه به ترتیب نسبت به آنتی‌بیوتیک‌های کوتریموکسازول و سفپیم مشاهده شد. 5/14 درصد از سویه‌های کلبسیلاپنومونیه و 5/15 درصد از سویه‌های اشریشیاکلی در روش فنوتیپی، مولد آنزیم متالوبتالاکتاماز بودند. هم‌چنین 9/88 درصد از سویه‌های اشریشیاکلی مورد بررسی حامل ژن SPM، 40 درصد حامل ژن VIM، 7/66 درصد حامل ژن SIM و 6/15 درصد حامل ژن IMP بودند. در سویه‌های کلبسیلاپنومونیهفراوانی ژن‌های SPM، VIM، SIM، IMP و GIM به ترتیب 8/54، 9/41، 1/58، 2/3 و 1/8 درصد مشاهده شد. نتیجه‌گیری: در این مطالعه درصد بالایی از ژن‌های مولد آنزیم متالوبتالاکتاماز در باکتری‌ها وجود داشتند، در حالی که 5/14 درصد از سویه‌های کلبسیلاپنومونیه و 5/15 درصد از سویه‌های اشریشیاکلی در روش فنوتیپی، مولد آنزیم متالوبتالاکتاماز بودند، این مسئله بیانگر این مطلب است که ژن‌های مولد آنزیم‌ها در باکتری‌ها وجود دارند و در مقادیر بالایی بیان نمی‌شوند. این احتمال نیز وجود دارد که این ژن‌ها بیان می گردند، اما با روش‌های فنوتیپی معمول قابل بررسی نیستند، لذا تلاش‌هایی در جهت یافتن روش‌های فنوتیپی قابل اطمینان، ارزان و آسان برای بررسی فنوتیپی باید صورت گیرد.  کلبسیلا پنومونیه، اشریشیاکلی، مقاومت متالوبتالاکتامازی، دیسک ترکیبی، ژن‌های SPM، VIM، SIM، IMP و GIM، واکنش زنجیره‌ای پلیمراز(PCR) Background & aim:  The use of broad-spectrum cephalosporins in the treatment of infections has led to the emergence of a new class of beta-lactamases called metallobetalactamase, these enzymes belong to Ambler's class B beta-lactamases. Group A and B metallobetalactamases in Enterobacteriaceae cause problems in treatment. The present study was conducted with the aim of phenotypic investigation of Escherichia coli and Klebsiella pneumoniae metallobetalactamase producing strains, as well as molecular investigation of SPM, VIM, SIM, IMP and GIM genes. <pre style="text-align:justify"> Methods: The present descriptive-quantitative study was conducted on 185 patients hospitalized in the special care units of Milad Hospital in Tehran from March to May 2018-2019, and 45 strains of Escherichia coli and 62 strains of Klebsiella pneumoniae were selected. In order to check the antibiotic resistance of the strains, diffusion test in agar was used and also to check the phenotypic of the metallobetalactamase producing strains, the combined disc method was used. Molecular investigation of metallobetalactamase producing genes was done by PCR method. The collected data were analyzed using Fisher's test. </pre> Results: In the present study, the highest antibiotic resistance of Escherichia coli and Klebsiella pneumoniae strains was observed to cotrimoxazole and cefepime antibiotics, respectively. 14.5% of Klebsiella pneumoniae strains and 15.5% of Escherichia coli strains produced metallobetalactamase enzyme in the phenotypic method. Also, 88.9% of the studied Escherichia coli strains carried the SPM gene, 40% carried the VIM gene, 66.7% carried the SIM gene, and 15.6% carried the IMP gene. SPM, VIM, SIM, IMP and GIM genes were found to be 54.8, 41.9, 58.1, 3.2 and 8.1% in Klebsiella pneumoniae strains, respectively. Conclusion: In the present study, there was a high percentage of genes producing metallobetalactamase enzyme in bacteria, while 14.5% of Klebsiella pneumoniae strains and 15.5% of Escherichia coli strains were producing metallobetalactamase enzyme in the phenotypic method. that genes producing enzymes exist in bacteria and are not expressed in high amounts.
