fa نقش گایدوایر در تسهیل عمل یورتروتومی‌داخلی با دید مستقیم تخصصي Special پژوهشي Research زمینه و هدف: تنگی مجرای ادرار در مردان یک مشکل شایع است. عمل اینترنال یورتروتومی‌با دید مستقیم، یکی از روش‌های شایع درمان در این بیماری است. هدف این مطالعه بررسی نقش گایدوایر در تسهیل عمل یورتروتومی داخلی با دید مستقیم بود. روش بررسی: در این مطالعه آینده نگر، 84 بیمار با تنگی مجرای قدامی ‌به صورت تصادفی به دو گروه مساوی تقسیم شدند. گروه اول به روش روتین تحت عمل یورتروتومی‌داخلی قرار گرفتند و در گروه دوم ابتدا با استفاده از یورتروسکوپ ولف 8 ـ 8/9 فرنچ، گایدوایر 035/0 فرنچ از سر نرم آن وارد مثانه شده و سپس اقدام به عمل یورتروتومی‌داخلی شد. داده‌ها با آزمون آماری توصیفی تجزیه و تحلیل شدند. یافته‌ها: متوسط زمان عمل در گروه اول 14 دقیقه و در گروه دوم، 10 دقیقه بود. مقایسه عوارض اولیه بعد از عمل نشان داد که در گروه اول در4 بیمار(52/9 درصد) خونریزی و عفونت تب‌دار در3 بیمار(14/7 درصد) رخ داد، ولی در گروه دوم چنین عوارضی مشاهده نشد. تنها عارضه دیرس در هر دو گروه عود تنگی مجرا بوده است که در گروه اول در 8 بیمار(04/19 درصد) و در گروه دوم در 3 بیمار(14/7 درصد) دیده شد. نتیجه‌گیری: استفاده از گایدوایر در جریان عمل یورتروتومی‌ممکن است هم باعث تسهیل عمل شود و زمان عمل را کوتاه‌تر کند و هم عوارض کوتاه مدت و دراز مدت عمل را کاهش دهد. Background & aim: Urethral Stricture (US) is a common urolologic problem. Direct optic internal urethratomy (DOIU) is a common option for treating US. The purpose of this study was to investigate the role of the guide wire to facilitate direct vision internal in adult male patients. Methods: In this prospective study, 84 patients with anterior urethral stricture randomly divided into two groups. The first group underwent routine DOIU surgery and the second group internal urethratomy was carried out using Wolf ureteroscope 8-9.8 French and guide wire 0.035 French which inserted into bladder. Data were collected and analyzed using software (spss). Results: The mean operation time was 14 minutes in group one and for group 2 was 10 minutes. The comparison of complications after surgery showed that in the first group urethral bleeding accrued in 4 patients (9.52%) and fever infection was seen in 3 patients (7.14%) respectively but no such events were observed in group 2. The only late complication in both groups was recurrence of the stricture that was seen in 8 patients (19.04%) of group one and 3 patients (7.14%) of group 2.
