کاربرد بی حسی نخاعی جهت انجام عمل نفرولیتوتریپسی پوستی

هدف و مقدمه: نفرولیتوتریپسی پوستی درمان انتخابی برای سنگ های کلیه با اندازه بالاتر از 20 تا 30 میلی متر، سنگ های شاخ گوزنی و همچنین سنگ هایی است که به صورت متعدد بوده و یا نسبت به درمان سنگ شکن سرپایی برون اندامی مقاوم می باشند. نفرولیتوتریپسی پوستی در اغلب مواقع تحت بیهوشی عمومی انجام می گیرد، در حالی که عوارض و هزینه بیهوشی عمومی بیشتر از بی حسی نخاعی می باشد، لذا هدف از این مطالعه بررسی کاربرد بی حسی نخاعی جهت انجام عمل نفرولیتوتریپسی پوستی می باشد.
مواد و روش ها: این مطالعه توصیفی به صورت مجموعه موارد طی سال های 1387ـ1385 بر روی 120 بیمار با تشخیص سنگ کلیه در اتاق های عمل بیمارستان شهید بهشتی دانشگاه علوم پزشکی یاسوج انجام گردید. متغیرهای بیهوشی و جراحی، حین و پس از عمل در بیمارانی که تحت عمل نفرولیتوتریپسی پوستی با بی حسی نخاعی قرار گرفتند بررسی شدند. داده های جمع آوری شده با نرم افزار SPSS و آزمون مجذور کای آنالیز گردید.
یافته ها: میانگین زمان عمل 98±45 دقیقه و میانگین اندازه سنگ 3.1±1.8 سانتی متر بود. متوسط زمان برگشت حس و حرکت به ترتیب 135±22.7 دقیقه و 112±35.7 دقیقه بود. در تمام بیماران عمل نفرولیتوتریپسی پوستی با موفقیت انجام شده و هیچ گونه عارضه مهم و جدی از جمله صدمات عروقی یا احشایی و نیز خون ریزی غیر معمول در حین یا پس از عمل در هیچ یک از بیماران رخ نداد. در 90 نفر (75 درصد) از بیماران، در سونوگرافی پیگیری هیچ گونه سنگی دیده نشد و یا سنگ های باقیمانده کمتر از 5 میلی متر بودند.
نتیجه گیری: بی حسی نخاعی یک روش ایمن، موثر و مناسب برای انجام عمل نفرولیتوتریپسی پوستی بوده و می تواند جایگزین خوبی برای بیهوشی عمومی در بالغینی باشد که تحت این عمل قرار می گیرند.
Introduction & Objective:: PCNL is the treatment of choice for
renal calculi larger than 20-3o millimeter, staghorn stones and
stones that are multiple or resistant to ESWL. The aim of this study
was the evaluating of the impact of spinal anesthesia on
intraoperative and postoperative outcome in patients undergoing
PCNL.
Material & Methods: The intraoperative and postoperative
anesthetic and surgical variables were evaluated for hundred and
twenty consecutive patients underwent PCNL. Anesthetic and
surgical variables during and after operation in patient undergoing
PCNL under spinal anesthesia were recorded and collected data
were analyzed by SPSS software using chi square test.
Results: Mean time of operation from the beginning of anesthesia
to the end of operation was 98±45 minutes. Mean stone size was
3.1±1.8 cm. Mean time for return of sensory and motor activity
were135±22.7 and 112±35.7 minutes respectively. Major
intraoperative or postoperative complications such as visceral or
vascular injury and unusual bleeding did not occur in any of
patients. Five patients needed transfusion of 1 unit pack cell and
four patients complain from mild to moderate headache and
dizziness and also mild low back pain for 2-4 days after operation
that improved with analgesics and bed rest.
Conclusion: Spinal anesthesia is safe and effective and also
comfortable for performing PCNL and is a good alternative for
general anesthesia in adult patients.
