به علت شدت تنگی، به صورت ابتکاری طی ده سال گذشته با نفروستومی از راه پوست در دوره نوزادی تحت درمان قرار داده ایم، مورد بررسی قرار دهیم تا پیامد این اقدام ساده و کم خطر را در&nbsp; حفظ رشد و عملکرد کلیه نشان دهیم.</span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">روش بررسی </span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">این مطالعه به صورت گذشته نگر و از طریق بررسی پرونده های بیمارستانی و پیگیریهای بالینی بیمارانی که از دی ماه 1393 لغایت آذر ماه 1403 در دوره نوزادی تحت نفروستومی از راه پوست قرار گرفته اند، انجام می شود.عمده این بیماران در دوران جنینی با شک به تنگی مادرزادی محل اتصال لگنچه به حالب تشخیص داده شدند و در هفته اول تولد با انجام سونوگرافی مجدد تشخیص آنها مورد تایید قرار می گرفت. همچنین در هفته اول تولد با توجه به یافته های سونوگرافی، اسکن هسته ای دی تی پی ا (</span>DTPA<span arial="" lang="FA" style="font-family:">)،بمنظور تعیین و افتراق میزان&nbsp; عملکرد کلیه سالم و معیوب&nbsp; صورت می گرفت. موارد مشکوک به برگشت ادراری و یا موارد دوطرفه ونیز بیمارانی که والدین آنها حاضر به انجام این روش درمانی نبودند، در این مطالعه وارد نشدند. طول مدت نفروستومی4 الی 8هفته بوده است و ارزیابی نهایی با انجام اسکن هسته ای مجدد و مقایسه آن با اسکن پایه پس از این دوره، بوده است.</span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">یافته ها</span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">در این مطالعه 12 شیرخوار شامل 8 پسر شرکت داشته اند که تحت بی حسی موضعی عمل نفروستومی آنها توسط رادیولوژیست مداخله ای انجام می شد. مقایسه اسکن هسته ای قبل و بعد از این اقدام جراحی ساده، در جدولی مشخص گردیده است.</span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">نتیجه گیری </span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span style="direction:rtl"><span style="unicode-bidi:embed"><span calibri="" style="font-family:"><span arial="" lang="FA" style="font-family:">&nbsp;با توجه به اثر مخرب انسداد بر عملکرد کلیه ها به نظر میرسد در شرایط سنی نوزادی و شیرخوارگی، می توان با مداخله جراحی هر چه زودتر و رفع انسداد،&nbsp; Although the effective and definite treatment for this congenital anomaly is surgical intervention, but due to possibility of gradual renal function improvement in some cases, usually the politic of post ponding the operation is suggested. On the other hand, it is mentioned that a significant part of the kidney growth is happen in the first two years of life. To use this golden time, it seems that it&rsquo;s better to open the way as soon as possible and percutaneous nephrostomy (PCN) will be a simple and minimally invasive technique for this purpose. We have evaluated a group of newborns with severe UPJO who underwent PCN, when they were in the neonatal period, to estimate the effect of intervention time on the outcome of differential renal function of the affected kidney with severe UPJO.</span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span aptos="" style="font-family:">Methods</span></span></span><br> <span style="font-size:11pt"><span style="line-height:107%"><span aptos="" style="font-family:">This study was done on patients who underwent PCN in neonatal period from 2014 till 2024. The patients who were suspected for vesico-ureteral reflux, and cases with bilateral UPJO were excluded. Almost all of the patients were diagnosed in the intrauterine period by ultrasound protocol ordered by guidelines and re-evaluated by the first week ultrasound and confirmed by the first week DTPA renal scanning. The duration of nephrostomy was <span style="color:red">4 to 8 </span>weeks, and the final evaluation was based on the second DTPA scanning and its comparison with the baseline DTPA scanning.
