<strong><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: black;">زمینه و هدف:</span></span></span></strong><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: black;"> بیماری لوپوس یک بیماری خود ایمنی است که به ‏دنبال آن امکان درگیری ارگان‏های مختلف وجود خواهد داشت. علت بروز آن تاکنون به‏ طور کامل شناخته نشده است، هر چند تأثیر عواملی مانند عوامل محیطی، ژنتیکی و هورمونی در بروز آن مشخص شده است</span></span></span><span dir="LTR"><span style="font-size: 10pt;"><span style="color: black;"><span style="font-family: calibri;">.</span></span></span></span><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: black;"> هدف از این مطالعه، گزارش یک مورد از تظاهر</span></span></span> <span style="font-size: 10pt;"><span style="font-family: yagut;">لوپوس به دنبال زنبور گزیدگی بود.</span></span><br>

<strong><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: black;">روش کار: </span></span></span></strong><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: red;">این مطالعه یک گزارش موردی است که </span></span></span><span style="font-size: 10pt;"><span style="font-family: yagut;"><span style="color: black;">اطلاعات مربوط به بیمار از پرونده و معاینههای بالینی جمعآوری شده و در پرسشنامه تهیه شده وارد شد. نتایج آزمایشهای بیمار نیز مورد بررسی قرار گرفت.</span></span></span><br>

<strong><span style="font-size: 10pt;"><span style="font-family: yagut;">معرفی بیمار:</span></span></strong><span style="font-size: 10pt;"><span style="font-family: yagut;"> کودکی 11 ساله به ‏دلیل بروز علایمی شامل؛ تب، راش پروانه‏ای صورت و درد مفاصل از 20 روز قبل، در بیمارستان بستری گردید. کودک هیچ علامت خاصی نداشته است تا این که دچار زنبور گزیدگی در ناحیه گردن می‏گردد و بعد از دو هفته از گزش زنبور علایم به‏ صورت راش ژنرالیزه، راش پروانهای صورت، آرتریت و حساسیت به نور آفتاب بروز می‏کنند. در آزمایشهای انجام شده وجود آنمی، مثبت بودن آنتی‏بادی ضد هسته‏ای و آنتی‏بادی علیه </span></span><span dir="LTR"><span style="font-size: 8pt;"><span style="font-family: arial,sans-serif;">DNA</span></span></span><span style="font-size: 10pt;"><span style="font-family: yagut;"> دو رشته‏ای و وجود پروتیین در ادرار مشاهده گردید. انجام نمونه‏برداری از کلیه، نشان دهنده وجود نفریت ناشی از لوپوس کلاس </span></span><span dir="LTR" style="background: white;"><span style="font-size: 8pt;"><span style="font-family: arial,sans-serif;"><span style="color: black;">III</span></span></span></span><span style="font-size: 10pt;"><span style="font-family: yagut;"> در بیمار بوده است. برای بیمار درمان پردنیزولون و هیدروکسی‏کلروکین انجام شده و در پیگیری، بیماری کنترل گردیده و دوز دارو کاهش یافت.</span></span><br>

<strong><span style="font-size: 10pt;"><span style="font-family: yagut;">نتیجهگیری:</span></span></strong><span style="font-size: 10pt;"><span style="font-family: yagut;"> بر اساس این گزارش و سایر مطالعهها، به‏نظر میرسد که سم زنبور دارای اثری دو‏طرفه بر سیستم ایمنی می‏باشد. هرچند در برخی از گزارش‏ها از سم زنبور جهت درمان بیماری‏های خودایمنی مانند آرتریت استفاده شده است، اما از طرفی به ‏دلیل تأثیر سم زنبور بر سیستم ایمنی، گاهی بیماری‏های خود ایمنی مانند: سندرم گیلنباره، هنخ شونلاین پورپورا، میوکاردیت و نفریت بروز پیدا کرده‏اند. اگرچه تاکنون به‏جز یک مورد از بروز لوپوس پوستی تحت حاد به ‏دنبال گزش زنبور، گزارش دیگری مشاهده نشده است. بنابراین با توجه به ‏این که در کنار اثرات درمانی سم زنبور، امکان ایجاد واکنش‏های ایمونولوژیک و بروز بیماری نیز وجود دارد، انجام مطالعههای بیشتر و بازبینی در استفاده از آن در درمان ضروری به‏نظر می‏رسد.</span></span><br>

<strong>Abstract</strong><br>
<strong>Background and objectives:</strong> <strong>Systemic Lupus Erythematosus (SLE) is an autoimmune disease that affects multiple organs. The etiology of SLE is not exactly defined</strong>. However, genetic and environmental factors, hormones and infections have been identified as triggers for SLE. Several organ systems can be damaged following SLE. In the present study, a case of SLE, triggered by bee sting and presented with renal involvement is reported.<br>
<strong>Methods: This study presents a case report of SLE, triggered by bee sting. Data gathering sheet was prepared. Demographic information, clinical and lab data were observed and medical records were considered. Informed consent was obtained.</strong><br>
<strong>Patient presentation:</strong> An <a name="_GoBack"></a>11-year-old boy was admitted to the hospital with the history of fever, malar rash and joint pain, presented from 20 days ago. The child had no presentations of the disease, until he was stung by a bee on his neck. Two weeks later, he developed generalized rash, malar rash, arthritis and photosensitivity. In lab data; anemia, proteinuria, increased levels of anti-nuclear antibody, anti-double strand DNA and erythrocyte sedimentation rate were observed. Class III of nephritis was observed based on the renal biopsy. Treatment with prednisolone and hydroxychloroquine was started and in one year of follow up, favorable outcomes were observed.<br>
<strong>Conclusions:</strong> Based on the presented case and the previous reports, it seems that bee venom might play a dual role in the immune system. Although the beneficial effects of bee venom have been observed in some autoimmune diseases (arthritis), some of the autoimmune diseases have been triggered by bee venom (such as Guillain-Barre syndrome, Henoch-Shonleine purpura, myocarditis and nephritis). This study is of the few studies, reporting SLE following bee sting. So, considering the reported adverse effects of bee venom, the application of bee venom in the treatment of diseases should be reconsidered.<br>
